Pre Visit Checklist
We will need you to arrive 15 minutes prior to your appointment time with:
• Picture ID
• Medical & Vision Insurance cards*
• Insurance Responsible parties Name, DOB, & SSN
• Copays are due at time of service
*if no vision card was given to you please contact your Human Resources Department or Medical Insurance company to obtain the name of your vision plan
Rose Optical does have a No Show charge for missed appointments, we ask that you contact the office to cancel or reschedule to avoid this fee!
